Consumer-Directed (CD) Services allow eligible Medicaid members to have greater choice and control over their long-term care. Rather than having a home care agency assign caregivers, you can select the people you trust to assist with your daily needs while receiving support to manage the service.
What Is Consumer-Directed Services?
Consumer-Directed Services give you the flexibility to decide who provides your care, when care is provided, and how your approved services fit into your daily life. If you qualify for a Medicaid waiver that includes Consumer-Directed Services, you may choose your own caregiver, including, in many cases, a trusted family member or friend who meets program requirements. While you remain in charge of your care, a Service Facilitator provides ongoing guidance to help you understand program requirements, develop your care plan, and maintain the documentation needed to remain in compliance with Medicaid guidelines.

Determine Eligibility
